Remove the following orphan declarations from drivers/misc/sgi-xp/xp.h:
1. xp_nofault_PIOR_target
2. xp_error_PIOR
3. xp_nofault_PIOR

They have been removed since commit 9726bfcdb977 ("misc/sgi-xp:
remove SGI SN2 support"), so remove them.

Remove the define is_uv() is_uv_system and just use the latter as is.
This removes a conflict with a new symbol in the generated uv_mmrs.h
file (is_uv()).

Note this also marks xp broken on ia64 now, as the UV support, which
was disable in generic kernels before actually never compiled due to
undefined uv_gpa_to_soc_phys_ram and uv_gpa_in_mmr_space symbols since
at least commit c2c9f1157414 ("x86: uv: update XPC to handle updated
BIOS interface").

Prepare to mark sensitive kernel structures for randomization by making
sure they're using designated initializers. In this case, no initializers
are needed (they can be NULL initialized and callers adjusted to check
for NULL, which is more efficient than an indirect call).
