The include/linux/pgtable.h is going to be the home of generic page table manipulation functions. Start with moving asm-generic/pgtable.h to include/linux/pgtable.h and make the latter include asm/pgtable.h. genwqe: convert get_user_pages() --> pin_user_pages()

This code was using get_user_pages*(), in a "Case 2" scenario (DMA/RDMA), using the categorization from [1]. That means that it's time to convert the get_user_pages*() + put_page() calls to pin_user_pages*() + unpin_user_pages() calls. There is some helpful background in [2]: basically, this is a small part of fixing a long-standing disconnect between pinning pages, and file systems' use of those pages. compat_ioctl: use correct compat_ptr() translation in drivers

A handful of drivers all have a trivial wrapper around their ioctl handler, but don't call the compat_ptr() conversion function at the moment. In practice this does not matter, since none of them are used on the s390 architecture and for all other architectures, compat_ptr() does not do anything, but using the new compat_ptr_ioctl() helper makes it more correct in theory, and simplifies the code. I checked that all ioctl handlers in these files are compatible and take either pointer arguments or no argument.
