md: add error_handlers for raid0 and linear

After the commit 9631abdbf406c("md: Set MD_BROKEN for RAID1 and RAID10") MD_BROKEN must be set if array is failed because state_store() checks it. If it is set then -EBUSY is returned to userspace. For raid0 and linear MD_BROKEN is not set by error_handler(). As a result mdadm is unable to trigger clean-up actions. It is a regression. This patch adds appropriate error_handler for raid0 and linear. The error handler sets MD_BROKEN for this device. Just use a non-zero max_discard_sectors as an indicator for discard support, similar to what is done for write zeroes. The only places where needs special attention is the RAID5 driver, which must clear discard support for security reasons by default, even if the default stacking rules would allow for it. Replace the gendisk pointer in struct bio with a pointer to the newly improved struct block device. From that the gendisk can be trivially accessed with an extra indirection, but it also allows to directly look up all information related to partition remapping. block: add a new revalidate_disk_size helper

revalidate_disk is a relative awkward helper for driver use, as it first calls an optional driver method and then updates the block device size, while most callers either don't need the method call at all, or want to keep state between the caller and the called method.
