Update indexer uses of ASSERT and ASSERT_LOG_ONLY to
VDO_ASSERT and VDO_ASSERT_LOG_ONLY, respectively. Remove
ASSERT and ASSERT_LOG_ONLY. Also rename uds_assertion_failed
to vdo_assertion_failed.
