In hfcsusb_probe(), the memory allocated for ctrl_urb gets leaked when
setup_instance() fails with an error code. Fix that by freeing the urb
before freeing the hw structure. Also change the error paths to use the
goto ladder style.

Compile tested only. Issue found using a prototype static analysis tool.

As &amp;hc-&gt;lock is acquired by both timer _hfcpci_softirq() and hardirq
hfcpci_int(), the timer should disable irq before lock acquisition
otherwise deadlock could happen if the timmer is preemtped by the hadr irq.

Possible deadlock scenario:
hfcpci_softirq() (timer)
    -&gt; _hfcpci_softirq()
    -&gt; spin_lock(&amp;hc-&gt;lock);
        &lt;irq interruption&gt;
        -&gt; hfcpci_int()
        -&gt; spin_lock(&amp;hc-&gt;lock); (deadlock here)

This flaw was found by an experimental static analysis tool I am developing
for irq-related deadlock.

The tentative patch fixes the potential deadlock by spin_lock_irq()
in timer.

In a future patch HAS_IOPORT=n will result in inb()/outb() and friends
not being declared. We thus need to add HAS_IOPORT as dependency for
those drivers using them. With that the !S390 dependency on ISDN can be
removed as all drivers without HAS_IOPORT requirement now build.

Remove pci_clear_master to simplify the code,
the bus-mastering is also cleared in do_pci_disable_device,

It is not allowed to call kfree_skb() or consume_skb() from hardware
interrupt context or with hardware interrupts being disabled.

skb_queue_purge() is called under spin_lock_irqsave() in handle_dmsg()
and hfcm_l1callback(), kfree_skb() is called in them, to fix this, use
skb_queue_splice_init() to move the dch-&gt;squeue to a free queue, also
enqueue the tx_skb and rx_skb, at last calling __skb_queue_purge() to
free the SKBs afer unlock.
