The regmap_read_poll_timeout() uses ADF4377_0000_SOFT_RESET_R_MSK
twice instead of checking both SOFT_RESET_MSK (bit 0) and
SOFT_RESET_R_MSK (bit 7). This causes an incomplete reset status check.

The code first sets both SOFT_RESET and SOFT_RESET_R bits to 1 via
regmap_update_bits(), then polls for them to be cleared. Since we set
both bits before polling, we should be waiting for both to clear.

Fix by using both masks as done in regmap_update_bits() above.

The ADF4350/1 features a programmable dual-modulus prescaler of 4/5 or 8/9.
When set to 4/5, the maximum RF frequency allowed is 3 GHz.
Therefore, when operating the ADF4351 above 3 GHz, this must be set to 8/9.
In this context not the RF output frequency is meant
- it's the VCO frequency.

Therefore move the prescaler selection after we derived the VCO frequency
from the desired RF output frequency.

This BUG may have caused PLL lock instabilities when operating the VCO at
the very high range close to 4.4 GHz.

Add separate handling for adf4378 within the driver.

The main difference between adf4377 and adf4378 is that adf4378 has only

Add clk provider feature for the adf4350.

Even though the driver was sent as an IIO driver in most cases the
device is actually seen as a clock provider.

This patch aims to cover actual usecases requested by users in order to
completely control the output frequencies from userspace.
