drm: Release driver tracking before making the object available again

commit fe4600a548f2763dec91b3b27a1245c370ceee2a upstream. This is the same bug as we fixed in commit f6cd7daecff5 ("drm: Release driver references to handle before making it available again"), but now the exposure is via the PRIME lookup tables. If we remove the object/handle from the PRIME lut, then a new request for the same object/fd will generate a new handle, thus for a short window that object is known to userspace by two different handles. Fix this by releasing the driver tracking before PRIME. Mostly this is unexpected indents. But really it's just a demonstration for my patch, all these issues have been found&fixed using the correct source file and line number support I just added. All line numbers have been perfectly accurate. One issue looked a bit fishy in intel_lrc.c, where I don't quite grok what sphinx is unhappy about. But since that file looks like it has never seen a proper kernel-doc parser I figured better to fix in a separate path. v2: Use fancy new &drm_device->struct_mutex linking (Jani). Finally all the core gem and a lot of drivers are entirely free of dev->struct_mutex depencies, and we can start to have an entirely lockless unref path. To make sure that no one who touches the core code accidentally breaks existing drivers which still require dev->struct_mutex I've made the might_lock check unconditional.
