Handle case when module is unloaded (kfd_exit) before a process space
(mm_struct) is released.

v2: Fixed potential race conditions by removing all kfd_process from
the process table first, then working on releasing the resources.

v3: Fixed loop element access / synchronization.  Fixed extra empty lines.

Remember KFD module initializaton status in a global variable. Skip KFD
device probing when the module was not initialized. Other amdgpu_amdkfd
calls are then protected by the adev-&gt;kfd.dev check.

Also print a clear error message when KFD disables itself. Amdgpu
continues its initialization even when KFD failed.

Add a folder structure to /sys/class/kfd/kfd/ called proc which contains
subfolders, each representing an active KFD process' PID, containing 1
file: pasid.

Since amdkfd is merged into amdgpu module and amdgpu can access amdkfd

Since KFD is only supported by single GPU driver, it makes sense to merge
amdgpu and amdkfd into one module. This patch is the initial step: merge
Kconfig and Makefile.
