Use dst input parameter to setup gart page table entries instead of using fixed

Binds pages that located in VRAM to the GART page table.

Useful when a kernel BO is located in VRAM but
needs to be accessed from the GART address space,
for example to give a kernel BO a 32-bit address
when GART is placed in LOW address space.

compiling with the latest upstream mainline GCC:

We need to take the reset domain lock before flush hdp. We can't put the
lock inside amdgpu_device_flush_hdp itself because it is used during
reset where we already take the write side lock.

This allows kernel mapped pages like the PDB and PTB to be
read via the iomem debugfs when there is no vram in the system.

Some AMD APUs may not have a dedicated VRAM. On such platforms the GART
table should be allocated on the system memory. When real vram size is
zero, place the GART table in system memory and create an SG BO to make
it GPU accessible.

As the layout of VMHUB definition has been changed to cover multiple
XCD/AID case, the original num_vmhubs is not appropriate to do vmhub
iteration any more.

Drop num_vmhubs and introduce vmhubs_mask instead.
