block: fold cmd_type into the REQ_OP_ space

Instead of keeping two levels of indirection for requests types, fold it all into the operations. The little caveat here is that previously cmd_type only applied to struct request, while the request and bio op fields were set to plain REQ_OP_READ/WRITE even for passthrough operations. Instead this patch adds new REQ_OP_* for SCSI passthrough and driver private requests, althought it has to add two for each so that we can communicate the data in/out nature of the request. The raw_cmd_copyin() function does a kmalloc() with GFP_USER, although the allocated structure is obviously not mapped to userspace, just copied from/to. In this case GFP_KERNEL is more appropriate, so let's use it, although in the current implementation this does not manifest as any error. block: bio: pass bvec table to bio_init()

Some drivers often use external bvec table, so introduce this helper for this case. It is always safe to access the bio->bi_io_vec in this way for this case. After converting to this usage, it will becomes a bit easier to evaluate the remaining direct access to bio->bi_io_vec, so it can help to prepare for the following multipage bvec support. Commit 09954bad4 ("floppy: refactor open() flags handling"), as a side-effect, causes open(/dev/fdX, O_ACCMODE) to fail. It turns out that this is being used setfdprm userspace for ioctl-only open(). Reintroduce back the original behavior wrt !(FMODE_READ|FMODE_WRITE) modes, while still keeping the original O_NDELAY bug fixed. For block drivers that specify a parent device, convert them to use device_add_disk(). This patch converts the simple bi_rw use cases in the block, drivers, mm and fs code to set/get the bio operation using bio_set_op_attrs/bio_op These should be simple one or two liner cases, so I just did them in one patch.
