Drop pointless VPRINTK() calls for entering routines and setting up sg
tables. And while we're at it, remove the disabled debugging messages.

The DPRINTK call doesn't print information which isn't already covered
by tracepoints later on.
Remove it.

In case a driver wants to return an error from qc_prep, return enum
ata_completion_errors. sata_mv is one of those drivers -- see the next
patch. Other drivers return the newly defined AC_ERR_OK.

[v2] use enum ata_completion_errors and AC_ERR_OK.

Use the dma_set_mask_and_coherent helper to set the DMA mask. Rely
on the relatively recent change that setting a larger than required
mask will never fail to avoid the need for the boilerplate 32-bit
fallback code.

In commit af7ddd8a627c
("Merge tag 'dma-mapping-4.21' of git://git.infradead.org/users/hch/dma-mapping"),
dmam_alloc_coherent has already zeroed the memory.
So memset is not needed.

None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.
