WSR and RSR are too generic and collide with other macro definitions in
the kernel causing warnings in allmodconfig builds. Drop WSR and RSR
macros and WSR_* and RSR_* variants. Change get_sr and set_sr to
xtensa_get_sr and xtensa_set_sr. Fix up users.

Commits 925f5532 (xtensa: ccount based clockevent implementation) and e3f43291
(xtensa: ccount based sched_clock) introduced users of ccount_freq. This
variable doesn't exist when CONFIG_XTENSA_CALIBRATE_CCOUNT is disabled. Add
ccount_freq definition in this case.

Add support for dispatching medium-priority interrupts, that is,
interrupts of priority levels 2 to EXCM_LEVEL. IRQ handling may be
preempted by higher priority IRQ.

- reference SRs by names where possible, not by numbers;
- get rid of __stringify around SR names where possible;
