tty: um: convert to u8/__u8 and size_t

Switch character types to u8 and sizes to size_t. To conform to characters/sizes in the rest of the tty layer. Note we use __u8 in the userspace files. This does a lot of cleanup on the UML console system. This patch should be entirely non-functional. Some locking documentation and a cleanup. uml_exitcode is copied into a local before sprintf sees it, in case sprintf does anything non-atomic with it. Initial git repository build. I'm not bothering with the full history, even though we have it. We can create a separate "historical" git archive of that later if we want to, and in the meantime it's about 3.2GB when imported into git - space that would just make the early git days unnecessarily complicated, when we don't have a lot of good infrastructure for it. Let it rip!
