sh: Replace ASSEMBLY with ASSEMBLER in all headers

While the GCC and Clang compilers already define __ASSEMBLER__ automatically when compiling assembly code, __ASSEMBLY__ is a macro that only gets defined by the Makefiles in the kernel. This can be very confusing when switching between userspace and kernelspace coding, or when dealing with uapi headers that rather should use __ASSEMBLER__ instead. So let's standardize on the __ASSEMBLER__ macro that is provided by the compilers now. This is a completely mechanical patch (done with a simple "sed -i" statement). Add PFN_PTE_SHIFT, update_mmu_cache_range(), flush_dcache_folio() and flush_icache_pages(). Change the PG_dcache_clean flag from being per-page to per-folio. Flush the entire folio containing the pages in flush_icache_pages() for ease of implementation. Most architectures (except arm64/x86/sparc) simply return 1 for kern_addr_valid(), which is only used in read_kcore(), and it calls copy_from_kernel_nofault() which could check whether the address is a valid kernel address. So as there is no need for kern_addr_valid(), let's remove it. sh/mm: enable ARCH_HAS_VM_GET_PAGE_PROT

This enables ARCH_HAS_VM_GET_PAGE_PROT on the platform and exports standard vm_get_page_prot() implementation via DECLARE_VM_GET_PAGE_PROT, which looks up a private and static protection_map[] array. Subsequently all __SXXX and __PXXX macros can be dropped which are no longer needed. mm: define default value for FIRST_USER_ADDRESS

Currently most platforms define FIRST_USER_ADDRESS as 0UL duplication the same code all over. Instead just define a generic default value (i.e 0UL) for FIRST_USER_ADDRESS and let the platforms override when required. This makes it much cleaner with reduced code. mm: consolidate pgtable_cache_init() and pgd_cache_init()

Both pgtable_cache_init() and pgd_cache_init() are used to initialize kmem cache for page table allocations on several architectures that do not use PAGE_SIZE tables for one or more levels of the page table hierarchy. Most architectures do not implement these functions and use __weak default NOP implementation of pgd_cache_init(). Since there is no such default for pgtable_cache_init(), its empty stub is duplicated among most architectures. Rename the definitions of pgd_cache_init() to pgtable_cache_init() and drop empty stubs of pgtable_cache_init().
