[ Upstream commit 9733862e50fdba55e7f1554e4286fcc5302ff28e ]

Commit f959dcd6ddfd29235030e8026471ac1b022ad2b0 (dma-direct: Fix
potential NULL pointer dereference) added a null check on the
dma_mask pointer of the kernel's device structure.

Add a dma_mask variable to the ps3_dma_region structure and set
the device structure's dma_mask pointer to point to this new variable.

Fixes runtime errors like these:
# WARNING: Fixes tag on line 10 doesn't match correct format
# WARNING: Fixes tag on line 10 doesn't match correct format

<title>powerpc: Delete non-required instances of include &lt;linux/init.h&gt;</title>

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

The one instance where we add an include for init.h covers off
a case where that file was implicitly getting it from another
header which itself didn't need it.

To allow more control of the verbosity of ps3_result() add a check
for the preprocessor macro PS3_VERBOSE_RESULT that builds a verbose
verion of the ps3_result() routine.

<title>ps3: Replace direct file operations by callback</title>

Currently the FLASH database is updated by the kernel using file operations,
meant for userspace only. While this works for us because copy_{from,to}_user()
on powerpc can handle kernel pointers, this is unportable and a bad example.
Replace the file operations by callbacks, registered by the ps3flash driver.

<title>powerpc/ps3: Use dma_addr_t down through the stack</title>

Push the dma_addr_t type usage all the way down to where the actual
values are manipulated.

Now that u64 is "unsigned long long", this removes warnings like:

arch/powerpc/platforms/ps3/system-bus.c:532: warning: passing argument 4 of 'ps3_dma_map' from incompatible pointer type
arch/powerpc/platforms/ps3/system-bus.c:649: warning: passing argument 4 of 'ps3_dma_map' from incompatible pointer type
