powerpc: Remove file parameter from phys_mem_access_prot()

Remove 'file' parameter from struct machdep_calls.phys_mem_access_prot and its implementation in pci_phys_mem_access_prot(). The file is not used on PowerPC. By removing it, a later patch can simplify fbdev's mmap code, which uses phys_mem_access_prot() on PowerPC. PCI: Remove pci_get_legacy_ide_irq() and asm-generic/pci.h

pci_get_legacy_ide_irq() is only used on platforms that support PNP, so many architectures define it but never use it. Replace uses of it with ATA_PRIMARY_IRQ() and ATA_SECONDARY_IRQ(), which provide the same functionality. Some functions defined in `arch/powerpc/kernel` (and one in `arch/powerpc/ kexec`) are deserving of an `__init` macro attribute. These functions are only called by other initialization functions and therefore should inherit the attribute. Also, change function declarations in header files to include `__init`. Split out all the bits that are purely for dma_map_ops implementations and related code into a new header so that they don't get pulled into all the drivers. That also means the architecture specific is not pulled in by any more, which leads to a missing includes that were pulled in by the x86 or arm versions in a few not overly portable drivers. powerpc/pci: Remove pcibios_setup_bus_devices()

With the previous patch applied pcibios_setup_device() will always be run when pcibios_bus_add_device() is called. There are several code paths where pcibios_setup_bus_device() is still called (the PowerPC specific PCI hotplug support is one) so with just the previous patch applied the setup can be run multiple times on a device, once before the device is added to the bus and once after. There's no need to run the setup in the early case any more so just remove it entirely.
