Revert "change alloc_pages name in dma_map_ops to avoid name conflicts"

This reverts commit 983e6b2636f0099dbac1874c9e885bbe1cf2df05 which is commit 8a2f11878771da65b8ac135c73b47dae13afbd62 upstream. It wasn't needed and caused a build break on s390, so just revert it entirely. MIPS/jazzdma: return error code from jazz_dma_map_sg()

The .map_sg() op now expects an error code instead of zero on failure. vdma_alloc() may fail for different reasons, but since it only supports indicating an error via a return of DMA_MAPPING_ERROR, we coalesce the different reasons into -EIO as is documented on dma_map_sgtable(). Split out all the bits that are purely for dma_map_ops implementations and related code into a new header so that they don't get pulled into all the drivers. That also means the architecture specific is not pulled in by any more, which leads to a missing includes that were pulled in by the x86 or arm versions in a few not overly portable drivers. dma-mapping: add a new dma_alloc_pages API

This API is the equivalent of alloc_pages, except that the returned memory is guaranteed to be DMA addressable by the passed in device. The implementation will also be used to provide a more sensible replacement for DMA_ATTR_NON_CONSISTENT flag. MIPS/jazzdma: decouple from dma-direct

The jazzdma ops implement support for a very basic IOMMU. Thus we really should not use the dma-direct code that takes physical address limits into account.
