change alloc_pages name in dma_map_ops to avoid name conflicts

After redefining alloc_pages, all uses of that name are being replaced. Change the conflicting names to prevent preprocessor from replacing them when it's not intended. vgacon, arch/*: remove unused screen_info definitions

A number of architectures either kept the screen_info definition for historical purposes as it used to be required by the generic VT code, or they copied it from another architecture in order to build the VGA console driver in an allmodconfig build. The mips definition is used by some platforms, but the initialization on jazz is not needed. Now that vgacon no longer builds on these architectures, remove the stale definitions and initializations. MIPS/jazzdma: return error code from jazz_dma_map_sg()

The .map_sg() op now expects an error code instead of zero on failure. vdma_alloc() may fail for different reasons, but since it only supports indicating an error via a return of DMA_MAPPING_ERROR, we coalesce the different reasons into -EIO as is documented on dma_map_sgtable(). Split out all the bits that are purely for dma_map_ops implementations and related code into a new header so that they don't get pulled into all the drivers. That also means the architecture specific is not pulled in by any more, which leads to a missing includes that were pulled in by the x86 or arm versions in a few not overly portable drivers. dma-mapping: add a new dma_alloc_pages API

This API is the equivalent of alloc_pages, except that the returned memory is guaranteed to be DMA addressable by the passed in device. The implementation will also be used to provide a more sensible replacement for DMA_ATTR_NON_CONSISTENT flag.
