Don't include cvmseg states into thread_status when
CONFIG_CAVIUM_OCTEON_CVMSEG_SIZE is not defined or 0.

Fix compile for kernel without this feature.

Only x86 has own release_thread(), introduce a new weak release_thread()
function to clean empty definitions in other ARCHs.

Having a stable wchan means the process must be blocked and for it to

All get_fs/set_fs calls in MIPS code are gone, so remove implementation
of it.  With the clear separation of user/kernel space access we no
longer need the EVA special handling, so get rid of that, too.
