mm: remove kern_addr_valid() completely

[ Upstream commit e025ab842ec35225b1a8e163d1f311beb9e38ce9 ] Most architectures (except arm64/x86/sparc) simply return 1 for kern_addr_valid(), which is only used in read_kcore(), and it calls copy_from_kernel_nofault() which could check whether the address is a valid kernel address. So as there is no need for kern_addr_valid(), let's remove it. mm: add dummy pmd_young() for architectures not having it

mips/mm: enable ARCH_HAS_VM_GET_PAGE_PROT

This enables ARCH_HAS_VM_GET_PAGE_PROT on the platform and exports standard vm_get_page_prot() implementation via DECLARE_VM_GET_PAGE_PROT, which looks up a private and static protection_map[] array. Subsequently all __SXXX and __PXXX macros can be dropped which are no longer needed. Protection map difference between RIXI and non RIXI cpus is _PAGE_NO_EXEC and _PAGE_NO_READ usage. Both already take care of cpu_has_rixi while setting up the page bits.
