docs: arm: convert docs to ReST and rename to *.rst

Converts ARM the text files to ReST, preparing them to be an architecture book. The conversion is actually: - add blank lines and identation in order to identify paragraphs; - fix tables markups; - add some lists markups; - mark literal blocks; - adjust title markups. At its new index.rst, let's add a :orphan: while this is not linked to the main index.rst file, in order to avoid build warnings. MIPS: store the appended dtb address in a variable

Instead of rewriting the arguments to match the UHI spec, store the address of a appended or UHI supplied dtb in fw_supplied_dtb. That way the original bootloader arugments are kept intact while still making the use of an appended dtb invisible for mach code. Mach code can still find out if it is an appended dtb by comparing fw_arg1 with fw_supplied_dtb.
