MIPS: BMIPS: Replace <linux/clk-provider.h> by <linux/of_clk.h>

The Broadcom BMIPS platform code is not a clock provider, and just needs
to call of_clk_init().

Hence it can include <linux/of_clk.h> instead of <linux/clk-provider.h>.

Converts ARM the text files to ReST, preparing them to be an
architecture book.

The conversion is actually:
 - add blank lines and identation in order to identify paragraphs;
 - fix tables markups;
 - add some lists markups;
 - mark literal blocks;
 - adjust title markups.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

The DT core will register "simple-bus" by default, so it is not necessary
for arch specific code to do so unless there are custom match entries,
auxdata or parent device. Neither of those apply here, so remove the call.

BCM6362 is a BMIPS4350 SoC which needs the same fixup as BCM6368 in order to

MIPS: store the appended dtb address in a variable

Instead of rewriting the arguments to match the UHI spec, store the
address of a appended or UHI supplied dtb in fw_supplied_dtb.

That way the original bootloader arugments are kept intact while still
making the use of an appended dtb invisible for mach code.

Mach code can still find out if it is an appended dtb by comparing
fw_arg1 with fw_supplied_dtb.

BCM63268.
