The recent io_remap_pfn_range() rework applied the static and inline
specifiers to the implementation of io_remap_pfn_range_pfn() on MIPS
Alchemy, mirroring the same change on other platforms. However, this
function is defined in a source file and that definition causes a
conflict with its declaration. Fix this by dropping the specifiers.

The only instances in which we customise this function are ones in which we
customise the PFN used.

Instances where architectures were not passing the pgprot value through
pgprot_decrypted() are ones where pgprot_decrypted() was a no-op anyway, so
we can simply always pass pgprot through this function.

Use this fact to simplify the use of io_remap_pfn_range(), by abstracting
the PFN via io_remap_pfn_range_pfn() and using this instead of providing a
general io_remap_pfn_range() function per-architecture.

We have a number of missing prototypes warnings for board_setup(),
alchemy_set_lpj() and prom_init_cmdline(), prom_getenv() and
prom_get_ethernet_addr(). Fix those by providing definitions for the
first two functions in au1000.h which is included everywhere relevant,
and including prom.h for the last three.

Lift the dma_default_coherent variable from the mips architecture code
to the driver core.  This allows an architecture to sdefault all device
to be DMA coherent at run time, even if the kernel is build with support
for DMA noncoherent device.  By allowing device_initialize to set the
-&gt;dma_coherent field to this default the amount of arch hooks required
for this behavior can be greatly reduced.

fixup_bigphys_addr is only provided by the alchemy platform.  Remove
all the stubs, and ensure we only call it if it is actually implemented.

Also don't bother implementing io_remap_pfn_range if we don't have to,
and move the remaining implementation to alchemy platform code.

The coherentio variable has previously been used as a boolean value,
indicating whether the user specified that coherent I/O should be
enabled or disabled. It failed to take into account the case where the
user does not specify any preference, in which case it makes sense that
we should default to coherent I/O if the hardware supports it
(hw_coherentio is non-zero).

Introduce an enum to clarify the 3 different values of coherentio &amp; use
it throughout the code, modifying plat_device_is_coherent() &amp;
r4k_cache_init() to take into account the default case.
