01cf9d524ff0 ("microblaze/PCI: Support generic Xilinx AXI PCIe Host Bridge
IP driver") removed pcibios calls to:

 pcibios_setup_bus_self()
 pcibios_setup_bus_devices()

Given that pcibios_fixup_bus() was the only caller of those functions they
have now become dead code (along with the functions they were calling in
turn), so they can be removed.

This is relatively esoteric, and knowing that we don't have it makes life
easier in some cases rather than just an eventual -EINVAL from
pci_mmap_page_range().

We can declare it <linux/pci.h> even on platforms where it isn't going to
be defined. There's no need to have it littered through the various
<asm/pci.h> files.

Replace the pci_resource_to_user() declarations in each arch that defines
HAVE_ARCH_PCI_RESOURCE_TO_USER with a single one in linux/pci.h.

Change the MIPS static inline implementation to a non-inline version so the
static inline doesn't conflict with the new non-static linux/pci.h
declaration.

No functional change intended.

For a long time all architectures implement the pci_dma_* functions using
the generic DMA API, and they all use the same header to do so.

Move this header, pci-dma-compat.h, to include/linux and include it from
the generic pci.h instead of having each arch duplicate this include.

a6c140969b46 ("Delete pcibios_select_root") removed pcibios_select_root().
But a7db50405216 ("PCI: remove pcibios_scan_all_fns()") added a few copies
back, probably with some incorrect merge conflict resolutions.

Remove the still-unused pcibios_select_root() definitions.

All users of arch/microblaze/include/asm/pci.h get it by including
include/linux/pci.h, which in turn includes <asm/pci.h> after it declares
struct pci_dev.

The forward declaration here is unnecessary, so remove it.
