Some but not all architectures provide set_dma_ops(). Move dma_ops
from struct dev_archdata into struct device such that it becomes
possible on all architectures to configure dma_ops per device.

This change allows us to pass DMA_ATTR_SKIP_CPU_SYNC which allows us to
avoid invoking cache line invalidation if the driver will just handle it
via a sync_for_cpu or sync_for_device call.

The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:
1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.
2. It brings safeness and checking for const correctness because the
attributes are passed by value.

When turning this from inline to an exported function I was a bit
over-eager and made it GPL only. This prevents the use of pretty much
all non-GPL PCI driver which is a bit over the top. Let's bring it
back in line with other architecture.

This patch adds the ability to the DMA direct ops to fallback to the IOMMU
ops for coherent alloc/free if the coherent mask of the device isn't
suitable for accessing the direct DMA space and the device also happens
to have an active IOMMU table.

Scott's patch 1c98025c6c95 "Dynamic DMA zone limits" changed
dma_direct_alloc_coherent() to start using dev->coherent_dma_mask.
That seems fair enough, but it exposes the fact that some of the drivers
we care about on IBM platforms aren't setting the coherent mask.
The proper fix is to have drivers set the coherent mask and also have
the platform code honor it.
For now, just restrict the dynamic DMA zone limits to the platforms that
need it.

The dma_get_required_mask() function is used by some drivers to
query the platform about what DMA mask is needed to cover all of
memory. This is a bit of a strange semantic when we have to choose
between IOMMU translation or bypass, but essentially what it means
is "what DMA mask will give best performances".
Currently, our IOMMU backend always returns a 32-bit mask here, we
don't do anything special to it when we have bypass available. This
causes some drivers to choose a 32-bit mask, thus losing the ability
to use the bypass window, thinking this is more efficient. The problem
was reported from the driver of following device:

This patch adds an override of that function in order to, instead,

for drivers to prefer this configuration.

FSL PCI cannot directly address the whole lower 4 GiB due to
conflicts with PCICSRBAR and outbound windows, and thus
max_direct_dma_addr is less than 4GiB. Honor that limit in
dma_direct_alloc_coherent().
Note that setting the DMA mask to 31 bits is not an option, since many
PCI drivers would fail if we reject 32-bit DMA in dma_supported(), and
we have no control over the setting of coherent_dma_mask if
dma_supported() returns true.

Platform code can call limit_zone_pfn() to set appropriate limits
for ZONE_DMA and ZONE_DMA32, and dma_direct_alloc_coherent() will
select a suitable zone based on a device's mask and the pfn limits that
platform code has configured.

This patch adds the support for to create a direct iommu "bypass"
window on IODA2 bridges (such as Power8) allowing to bypass iommu
page translation completely for 64-bit DMA capable devices, thus
significantly improving DMA performances.
Additionally, this adds a hook to the struct iommu_table so that
the IOMMU API / VFIO can disable the bypass when external ownership
is requested, since in that case, the device will be used by an
environment such as userspace or a KVM guest which must not be
allowed to bypass translations.

Commit 9adc5374 ('common: dma-mapping: introduce mmap method') added a
generic method for implementing mmap user call to dma_map_ops structure.
This patch converts ARM and PowerPC architectures (the only providers of
dma_mmap_coherent/dma_mmap_writecombine calls) to use this generic
dma_map_ops based call and adds a generic cross architecture
definition for dma_mmap_attrs, dma_mmap_coherent, dma_mmap_writecombine
functions.
The generic mmap virt_to_page-based fallback implementation is provided for
architectures which don't provide their own implementation for mmap method.

With module.h being implicitly everywhere via device.h, the absence

Now that the generic code has dma_map_ops set, instead of having a
messy ifdef & if block in the base dma_get_required_mask hook push
the computation into the dma ops.
If the ops fails to set the get_required_mask hook default to the
width of dma_addr_t.
This also corrects ibmbus ibmebus_dma_supported to require a 64
bit mask. I doubt anything is checking or setting the dma mask on
that bus.

The hook dma_get_required_mask is supposed to return the mask required
by the platform to operate efficently. The generic version of
dma_get_required_mask in driver/base/platform.c returns a mask based
only on max_pfn. However, this is likely too big for iommu systems
and could be too small for platforms that require a dma offset or have
a secondary window at a high offset.
Override the default, provide a hook in ppc_md used by pseries lpar and
cell, and provide the default answer based on memblock_end_of_DRAM(),
with hooks for get_dma_offset, and provide an implementation for iommu
that looks at the defined table size. Coverting from the end address
to the required bit mask is based on the generic implementation.
The need for this was discovered when the qla2xxx driver switched to
64 bit dma then reverted to 32 bit when dma_get_required_mask said
32 bits was sufficient.

The only reason to require a dma_ops struct is to see if it has
implemented set_dma_mask. If not we can fall back to setting the mask
directly.
This resolves an issue with how to sequence the setting of a DMA mask
for platform devices. Before we had an issue in that we have no way of
setting the DMA mask before the various low level bus notifiers get
called that might check it (swiotlb).
So now we can do:

And expect the right thing to happen with the bus notifiers get called
via platform_device_add.

This is used by Alsa to mmap buffers allocated with dma_alloc_coherent()
into userspace. We need a special variant to handle machines with
non-coherent DMAs as those buffers have "special" virt addresses and
require non-cachable mappings

The current check is wrong because it does not take the DMA offset intot
account, and in the case of a driver which doesn't actually support
64bits would falsely report that device as working.

implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

The former is no longer really accurate with the swiotlb case now
a possibility. I also move it into dma-mapping.h - it no longer
needs to be in dma.c, and there are about to be some more accessors
that should all end up in the same place. A comment is added to
indicate that this function is not used in configs where there is no
simple dma offset, such as the iommu case.

On an iMac G5, the b43 driver is failing to initialise because trying to
set the dma mask to 30-bit fails. Even though there's only 512MiB of RAM
in the machine anyway:

We should probably let it succeed if the available RAM in the system
doesn't exceed the requested limit.

This patch includes the basic infrastructure to use swiotlb
bounce buffering on 32-bit powerpc. It is not yet enabled on
any platforms. Probably the most interesting bit is the
addition of addr_needs_map to dma_ops - we need this as
a dma_op because the decision of whether or not an addr
can be mapped by a device is device-specific.

The implementation we just revived has issues, such as using a
Kconfig-defined virtual address area in kernel space that nothing
actually carves out (and thus will overlap whatever is there),
or having some dependencies on being self contained in a single
PTE page which adds unnecessary constraints on the kernel virtual
address space.
This fixes it by using more classic PTE accessors and automatically
locating the area for consistent memory, carving an appropriate hole
in the kernel virtual address space, leaving only the size of that
area as a Kconfig option. It also brings some dma-mask related fixes
from the ARM implementation which was almost identical initially but
grew its own fixes.

We need to swap these out once we start using swiotlb, so add
them to dma_ops. Create CONFIG_PPC_NEED_DMA_SYNC_OPS Kconfig
option; this is currently enabled automatically if we're
CONFIG_NOT_COHERENT_CACHE. In the future, this will also
be enabled for builds that need swiotlb. If PPC_NEED_DMA_SYNC_OPS
is not defined, the dma_sync_*_for_* ops compile to nothing.
Otherwise, they access the dma_ops pointers for the sync ops.
This patch also changes dma_sync_single_range_* to actually
sync the range - previously it was using a generous
dma_sync_single. dma_sync_single_* is now implemented
as a dma_sync_single_range with an offset of 0.

On PowerPC 4xx or other non cache-coherent platforms, we lost the
appropriate cache flushing in dma_map_sg() when merging the 32 and
64-bit DMA code (commit 4fc665b88a79a45bae8bbf3a05563c27c7337c3d,
"powerpc: Merge 32 and 64-bit dma code"). This restores it.

After Becky's work we can almost have different DMA offsets
between on-chip devices and PCI. Almost because there's a
problem with the non-coherent DMA code that basically ignores
the programmed offset to use the global one for everything.
This fixes it.

We essentially adopt the 64-bit dma code, with some changes to support
32-bit systems, including HIGHMEM. dma functions on 32-bit are now
invoked via accessor functions which call the correct op for a device based
on archdata dma_ops. If there is no archdata dma_ops, this defaults
to dma_direct_ops.
In addition, the dma_map/unmap_page functions are added to dma_ops
because we can't just fall back on map/unmap_single when HIGHMEM is
enabled. In the case of dma_direct_*, we stop using map/unmap_single
and just use the page version - this saves a lot of ugly
ifdeffing. We leave map/unmap_single in the dma_ops definition,
though, because they are needed by the iommu code, which does not
implement map/unmap_page. Ideally, going forward, we will completely
eliminate map/unmap_single and just have map/unmap_page, if it's
workable for 64-bit.
