Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have MODULE_LICENCE("GPL*") inside which was used in the initial
   scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

  GPL-2.0-only

In preparation to enabling -Wimplicit-fallthrough, mark switch cases
where we are expecting to fall through.

Notice that, in this particular case, I placed all the "Phase N - ..."
comments on the same line as its corresponding switch case. The same
way in which similar comments appear in drivers/scsi/ppa.c. This makes
it possible to place the "fall through" annotations at the bottom of
each switch case, which is what GCC is expecting to find.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

Some drivers are ADDing the scsi command's result bytes instead of ORing
them.

While this can produce correct results it has unexpected side effects.
