The CCDC can interleave fields into a single buffer when writing to
memory. Support it.

Add PHY routing configuration for both 3430 and 3630. Also add register bit
definitions of CSIRXFE and CAMERA_PHY_CTRL registers on OMAP 3430 and 3630,
respectively.

The OMAP3ISP_*_REG_OFFSET, OMAP3ISP_*_REG_BASE and OMAP3ISP_*_REG macros
are not needed. Remove them.
The only exception is the OMAP3ISP_HIST_REG_BASE address. Replace it
with the memory address received through platform resources.

Commit c49f34bc ("ARM: OMAP2+ Move SoC specific headers to be local to
mach-omap2") moved omap34xx.h to mach-omap2. This broke omap3isp, as it
includes omap34xx.h.
Instead of moving omap34xx to platform_data, simply add the two
definitions the driver needs and remove the include altogether.

The remaining drivers are mostly platform drivers. Name the
dir to reflect it.

It makes sense to latter break it into a few other dirs.
